* EVALUASI PROGRAM
Program bagi Bona ini akan selalu dievaluasi dan dimonitor menggunakan lembar evaluasi dan lembar monitoring yang dibuat saat perencanaan program (contoh lembar evaluasi dan lembar monitoring terlampir).
Evaluasi dan monitoring dilakukan ibu Bona sebagai manajer program dan secara berkala akan didiskusikan bersama terapis untuk melihat efektivitas dan kemajuan program.
